5th MDG Third Party Collection Program

The 5th Medical Group is mandated by the Department of Defense to bill third party insurance policies for all non-active duty beneficiaries receiving any clinical, lab, radiology, and pharmacy services at the 5th MDG.

It is important for all Air Force Global Strike Command and Team Minot Airmen here to familiarize themselves with the 5th MDG's third party collection program to ensure access to health care benefits.

The process begins with information being collected on a DD Form 2569, otherwise known as the Record of Other Health Insurance. As required by the defense department, all non-active duty beneficiaries are required to fill out a DD Form 2569 annually. This must be done in all cases, including instances where Tricare or any other health insurance policy is used.

Once the form has been completed for each beneficiary, patients will receive a blue TPC card. This will serve as proof of form completion and is also validated prior to receiving services at the 5th MDG.

Failure to comply could result in health care service delays or in some cases, denial of care here. The 5th MDG will continue to strive to give each and every patient the best health care services possible, therefore all funds collected through the TPC program are used to enhance the quality of care and services everyone receives.
