Medical Group to offer sports physicals

MINOT AIR FORCE BASE, N.D. --  The 5th Medical Group here will be offering sports physicals to children at the Family Practice Clinic July 9 from 7:30 a.m. to 12:30 p.m.

The physicals are by appointment only and will be available to those in 7th through 12th grade participating in sports for the 2011-2012 school year. Those wishing to schedule an appointment may do so by calling 723-5633.

Medical history forms must be completed prior to the appointment. These forms are available at the Minot School District website at www.minot.k12.nd.us. Select the activities page, then athletics, followed by the sports physical link on the left side of the page. The form is located as a downloadable file at the bottom of this page.
