Guidance on applying for USAF funded entitlements Published July 19, 2011 5th Comptroller Squadron MINOT AIR FORCE BASE, N.D. -- As immediate reaction steps conclude and longer-term plans emerge for families dealing with the evacuation, the documentation links below will provide the latest guidance on how to apply for United States Air Force funded entitlements. As processing guidance, pleas see the 'CPTS Limited Evac Slides' and 'LRS Evacuation HHG Storage and Movement Process Guidance' . These two will be the source for how the process will work at Minot and should be reference by unit comand their staff in briefing evacuees on what to do and who to contact. If there are any questions on these two specific attachments, please contact either CPTS or LRS respectively. Other attachments are provided for your reference. For processing travel, lodging, and per diem requests on behalf of DEPENDENTS of Military and DoD Civilians (appropriated fund), please see: - 'Unit CC Evacuation Certification.' is to be used as the initiating step in DEPENDENT travel/lodging/& per diem. - '5 BW CC Flood Order - Travel & Per Diem.' is provided for reference. For processing house-hold goods short distance moves and non-temp storage requests for active duty members and the dependents of active duty members, please see: - 'AF 150 for 2011 Evacuees HHG storage.' is to be used for the processing of HHG storage for active duty member and the dependents of active duty members. Please note that the AF 150 is normally used by the 5 CES Housing Office. However, it has been adapted for this process and evacuees will not need to go to the 5th Civil Engineer Squadron Housing Office to get this form completed. This form will be filled out at the unit level and will be sent to 5 LRS for processing. - '5 BW CC Flood Order - HHG.' is provided for reference. One final note, while there are some standard entitlements that are allowed for evacuees depending on their unique situations, the attached guidance is the closest thing we have to a standard process. As evacuees, the most important thing to do for this process is to get the dependent travel orders initiated and the HHG storage planned before the August 5 deadline so that you don't lose out on the opportunity.
